Name, Address, Date of Birth, Email, Telephone Number, Employment, Lifestyle, Medical History and Hobbies.
What is this data used for?
Your contact number is used to remind you of forthcoming/missed appointment. The information is recorded on the client consultation form, this form is the initial appointment and treatment.
Treatment notes are added during each appointment. This is on paper based and stored securely at the premises, your details are not stored on a computer.
Who has access to your data?
Only the therapist who will be giving your treatment.
Who is this data shared with?
The data is not shared with anyone.
How is your data stored?
On paper only in a secured folder at the therapy room. We do not collect any special data only what is mentioned above.
How long do you keep consultation forms
Forms are kept for seven years from the last treatment, or if the client specifically ask us to remove such records.
Can I ask for data to be removed or corrected?
Yes, please contact the therapy room to facilitate this.
